This is a list of forms that you will need access to during your training in our PhD program. Please see the appropriate handbook for a more in depth outline of the requirements and guidance for each of these milestones.
Timeline
Research Expectations Form (BME 598/599/698/699)
DUE: Each Semester
Send to: Keep a copy for yourself (doesn't need to be sent anywhere)
- Research Expectations Guidance Document
- Student Advisor Expectation Scale (not required but useful especially in your first semester)
Individual Development Plans
DUE: Each Fall
Send to: Discussed and signed by your faculty advisor each fall; usually discussed during your mentoring meeting in the spring)
Qualifying Exam
DUE: May 15th, First Year
- Qualifying Guidance Sheet for Faculty and Students
- Qual Timeline
- Non-Default Qual Timeline
- PhD Qualifying Exam Pre-Registration Form (Due October 1st; usually in RF1)
- PhD Qualifying Exam Registration From (Due February 1st, usually in RF2)
- Qualifying Exam Cover Sheet (to be filled out by the student)
- Qualifying Exam Assessment Rubric
- Post Exam Qualtrics Survey (one compiled survey to be completed by students exam committee)

Plan of Study Worksheets and Transfer Courses
DUE: June 1st, First Year
Annual Mentoring Meeting
DUE: Annually
If students complete a qualifying exam or preliminary exam during the year, that will satisfy this committee meeting requirement for that year.
